The disc seems oval instead of round, as the side of the optic nerve next the staphyloma becomes involved in it. As the disease begins to affect the region of the yellow spot, or the optic nerve itself, vision becomes more impaired. With the increased bulging of the sclerotica the danger arises that the retina may not accommodate its surface to that of the outer tunic, and that effusion may be formed between the two structures, giving rise to the condition known as separation of the retina. That such abnormal elongation of the eyeball should be a cause of myopia may be readily seen,--the lengthening of the optic axis giving rise to excess of refraction, so that the focus is formed in front of its normal position. But the affection, itself a cause of myopia, may be yet further increased in consequence of it. Any long-continued convergence of the optic axes, in accommodation for near objects, causes greater external pressure upon the globe from the action of the recti muscles, and at the same time the stooping position of the head assumed in reading and writing favors congestion of the vessels and increases the intra-ocular pressure. Both these influences promote the further yielding of the already weakened tissues, and thus lead to an increase of the staphylomatous bulging.
